Teams  are Not Placed in the Order of Competition Until Registration with Payment is Complete.

Forms must be filled out completely and faxed, mailed, or emailed by the event deadline.

Team Registration Form

 

Instructions:

1 Form per organization. See form for event pricing. Be sure to check event web pages for current pricing. Registrations with incorrect calculations will be returned. Crossovers are 1/2 price per participant, per additional routine.

 

 

Team Roster/Waiver

 

Instructions:

1 Form for each squad. This can also be used as a medical waiver for each squad if parent signatures are included. This is typically used when parents are readily available to sign. Please fill in Hoodie/Jacket sizes for National Championships.

Annual Medical Waiver

 

Instructions:

All New! Annual Medical Waiver for teams who attend more than one Xtreme Spirit competition each season! Just have parents fill this out and fax to us, we'll keep it on file for the entire season's worth of events.

1 Form for each participant if not using Team Roster/Waiver as Medical Waiver. This is typically used as a "send home and have parent's sign and return" form.

Individual Divisions Registration Form

 

Instructions:

1 Form per Individual. For event pricing please view each

event's webpage. Pricing is per participant per performance. Registrations with incorrect calculations will be returned.

 

Vendor Registration Form

 

Instructions:

Pricing is included on form. Vendors are welcome at all events which average 70 teams per event. All forms and payments are due in by the event deadline, no exceptions.

All registrations must include payment. Teams will not be placed in the Order of Competition until paid in full. Teams registering after the deadline must pay via credit card or money order. Checks will not be accepted after the registration deadline and payments cannot be made at the event. Xtreme Spirit does not refund registrations after the registration deadline. 50% refund of registrations fees paid are eligible until the registration deadline for your event or 100% credit towards another event. All registration changes will require the use of our online change form and a $50 processing fee will apply. Changes also cannot be made without payment. Changes cannot be called or emailed in unless the online form does not allow the change you are requesting. All events will run regardless of weather conditions. Please prepare for extra drive time in areas where unfavorable weather conditions are likely.
